Summary: In Purgatory, Cas is patching up Dean's most recent injuries using his hospital pants.  Maybe they've just kissed, maybe they are just about to, maybe Dean is just thinking about it while Cas has his hands full.
I really liked the tenderness between Dean and Cas in krystallce's orignal picture, so I hope I've captured some of that here, and I hope krystallce approves of this version!
