Brilliantly done. I found myself laughing at little bits of it and then being horrified by finding some of Kevin's drugged-up antics even remotely funny. It's easy to giggle at the way he was bouncing around in a panic, and even mistakenly shooting Sam, but I think it was more laughing at the absurdity of how messed up everyone was. The whole thing was, in actuality, absolutely horrifying, and terribly sad. I always hated the way poor Kevin was treated. They say he's family, and I'm sure they did feel that way about him in their own way, but even though they protected him, they were also the most harmful to him, and they treated him abysmally.
I loved how Gadreel's feeling were filtering through to Sam's because he was stretched so thin. Sam's injuries and Dean's Hellucinations kept the tension incredibly high, and I even found myself feeling a little sorry for Crowley. No one was left unscathed. Loved it!
